The Science of Living Longer: mTOR, NAD+, Senescence, and What Actually Works
Nobel laureate Venki Ramakrishnan — who spent his career mapping the ribosome and then turned to aging biology — explains the key longevity pathways, why the field is 'full of hype,' and what the evidence actually supports.
Venki Ramakrishnan spent decades mapping the ribosome — the molecular machine that builds every protein in every living cell. That work earned him the 2009 Nobel Prize in Chemistry. Then he turned his attention to the bigger question: why do those cells eventually break down? Why do we age at all?
His answer, laid out across dozens of lectures and interviews, is both exciting and sobering. The science of longevity has never been more advanced. And it has never been more prone to hype. "The aging field," he says plainly, "is full of hype."
What follows is his account of the key pathways — what the biology actually says, where the evidence is solid, and where the field has already burned hundreds of millions of dollars chasing mirages.
The Framework: Hallmarks of Aging
Before getting to individual pathways, Ramakrishnan grounds everything in what aging researchers call the Hallmarks of Aging — a framework first published in 2013 and expanded to nine (and now twelve) distinct processes that characterize the aging body.
The logic of a hallmark is simple: if eliminating it slows aging and amplifying it accelerates aging, it qualifies. The hallmarks include DNA damage, epigenetic changes, loss of protein homeostasis, nutrient sensing gone wrong, mitochondrial dysfunction, cellular senescence, stem cell exhaustion, and altered intercellular communication.
Every major longevity pathway maps onto one or more of these hallmarks. They are not independent — they feed each other. Fix one and you often move another. That interconnection is both the promise of the field and the source of its complexity.
mTOR and Rapamycin: The Nutrient Switch
One of the most reproducible findings in aging research is that reducing calories — caloric restriction — extends lifespan and healthspan across almost every organism studied: worms, flies, mice, and in some markers, humans. Animals on restricted diets don't just live longer; "older animals start resembling younger animals in terms of their physiology and their biomarkers."
The obvious question is whether you can get the benefits without actually eating less. And this is where mTOR enters.
"There are many important biochemical pathways that are affected by caloric restriction. One of them is the IGF-1, insulin growth hormone factor. And one of the darling drugs of the anti-aging community is rapamycin, which inhibits a pathway called the TOR pathway, which also senses nutrients — so reducing calories has a similar effect as this drug rapamycin."
mTOR (mechanistic target of rapamycin) is essentially a cellular sensor that detects whether nutrients are abundant. When nutrients are plentiful, mTOR stays active, pushing the cell toward growth and protein synthesis. When nutrients are scarce, mTOR quiets down — and the cell shifts into a more conserving, repair-oriented mode. Rapamycin mimics the scarce-nutrient state by blocking mTOR directly.
In mice, rapamycin reliably extends lifespan — one of the most robust findings in the field. The catch is significant:
Rapamycin was originally approved as a drug to prevent organ rejection in transplant patients — because it suppresses the immune system. Taking it long-term to slow aging means accepting ongoing immune suppression. "Whether you would want to give that sort of drug over a long period — it has other side effects. Would you take rapamycin for a long period in order to age perhaps more slowly? It's not clear."
Researchers are now exploring lower doses and intermittent protocols that might preserve the aging benefits while reducing immunosuppression. The biology is sound. The clinical path is still being mapped.
NAD⁺ and Its Precursors (NMN, NR)
NAD (nicotinamide adenine dinucleotide) is one of the most fundamental molecules in biology — a coenzyme involved in hundreds of metabolic reactions and critical for energy production. Its levels decline with age, and that decline correlates with many aspects of aging.
"NAD precursors like NMN and NR have been shown in mice to restore the levels of NAD to levels of younger mice, and mice treated with these precursors tend to seem healthier — they have slower loss of stem cells or muscle tissue. Although their lifespan hasn't increased much. But maybe that's what you want. You want to live healthier but not longer."
This is a crucial distinction that gets lost in the supplement marketing: the mouse data on NAD precursors shows improved healthspan more clearly than extended lifespan. Better muscle function, fewer senescent cells, more vigorous tissue repair — these are real effects in mice. Whether they translate to humans at meaningful doses is what clinical trials are designed to determine.
What has not waited for those trials is the market. NAD precursor supplements — primarily NMN and NR — are already a business worth hundreds of millions of dollars annually and projected to exceed a billion. Ramakrishnan notes, with some wry amusement, that he personally knows two Nobel laureates who are already taking them. "That just shows that they're no more rational than anybody else."
What NAD precursors do
Boost cellular NAD levels. NAD is required for sirtuins (longevity enzymes) and PARP (DNA repair). Replenishing it in aged tissue may restore some youthful metabolic function.
State of the evidence
Strong mouse data for healthspan. Early human trials show NAD levels rise with supplementation. Whether that translates to meaningful health outcomes in humans is still under investigation.
Sirtuins and Resveratrol: A $750 Million Cautionary Tale
This is the section Ramakrishnan returns to most pointedly — a story about how legitimate science can be captured by hype, and how the longevity field has already paid a significant price for it.
Sirtuins are a family of enzymes that respond to NAD levels and regulate aging-related processes — gene expression, DNA repair, stress resistance. The original research linking them to extended lifespan was published in prestigious journals and generated enormous excitement. Resveratrol, a compound found in red wine, was proposed as a sirtuin activator.
"The whole concept was sold to Glaxo Wellcome for $750 million. And after many years of follow-up work, Glaxo Wellcome basically shut down that division."
What happened? Subsequent studies contradicted the original findings. The reported activation of sirtuins by resveratrol turned out to be an artifact of the measurement method. The sirtuin-lifespan link that had been established in simpler organisms did not hold up in mammals in the same way. The science was not fraudulent — the original researchers were working in good faith — but it was overinterpreted, and industry moved on it before the picture was clear.
"Science is a very rational endeavor, but scientists are humans and therefore not completely rational. There has been, as I warn, examples of sort of snake oil — things that were touted, even companies that were exchanged with hundreds of millions of dollars at stake, where none of it actually played out."
Sirtuins may still have a role in aging biology. The story isn't closed. But the speed at which the field moved from early mouse data to a three-quarter-billion-dollar acquisition is a calibration point for how to read longevity headlines.
Cellular Senescence and Senolytics
One of the most intriguing and genuinely promising areas of longevity research involves a strange class of cells that have stopped dividing but refuse to die. These are senescent cells — sometimes called "zombie cells" — and they accumulate with age.
"Senescence evolved for good reasons. For example, if a cell has damaged DNA, you don't want that cell hanging around and dividing because it might become cancerous. You can easily afford to get rid of one cell. But as you accumulate more and more of these cells, they start to contribute to inflammation and dysfunction."
The evolutionary logic is clean: senescence is a cancer prevention mechanism. A cell with dangerous DNA damage gets locked out of the cell cycle before it can proliferate. But over decades, senescent cells accumulate faster than the body clears them. They secrete inflammatory signals — a phenomenon called the senescence-associated secretory phenotype (SASP) — that poison the tissue around them and accelerate aging in neighboring cells.
This is where senolytics come in: drugs specifically designed to selectively kill senescent cells while leaving healthy cells intact.
Senolytics are already in human clinical trials. The concept is mechanistically sound, the animal data is compelling, and the therapeutic goal is clear. Ramakrishnan identifies this as one area where the scientific basis is solid and the clinical path is actually being pursued.
Mitochondria: The Aging Power Plants
Mitochondria are among the most extraordinary structures in biology — organelles that were once free-living bacteria, engulfed by an ancestral cell two billion years ago and never left. Today they are the cell's energy factories, converting oxygen and glucose into ATP, the universal energy currency.
With age, mitochondria accumulate damage, become less efficient, and produce more reactive oxygen species (ROS) — molecules that further damage cellular components including DNA. Mitochondrial dysfunction is one of the core hallmarks of aging, contributing to the energy deficits, muscle weakness, and metabolic deterioration that characterize the aging body.
Caloric restriction and exercise are the two most reliable interventions for maintaining mitochondrial health — both increase the production of new mitochondria (mitophagy clears old ones) and improve their efficiency. Many longevity drugs, including rapamycin, influence mitochondrial function indirectly through the pathways they target.
Epigenetics: The Program That Ages
The DNA sequence itself doesn't change much as we age (beyond accumulated mutations). What changes is how that DNA is read — which genes are expressed, which are silenced, and how the whole program of cell identity is maintained.
"The DNA program itself ages. Damage and modifications change the nature of the genes that you're expressing, and that results in the proteins which are encoded by those genes being expressed differently."
These modifications — primarily methylation of DNA and modification of the histones that DNA wraps around — are collectively called the epigenome. Researchers including Steve Horvath have developed "epigenetic clocks" that can predict biological age more accurately than chronological age. Crucially, epigenetic age is reversible in some contexts: the Yamanaka factors, which can reprogram adult cells back to a stem-like state, reset the epigenetic clock. Whether this can be done safely in living organisms is an active research frontier.
Young Blood: Legitimate Science, Instant Exploitation
The parabiosis experiments — where old and young mice are surgically connected to share a bloodstream — produced some of the most striking results in aging research. The old mouse benefits. The young mouse deteriorates. Something in blood composition changes with age, and those changes can be partially reversed.
"It's not incredibly successful, but the science underlying it is solid. If you connect an old rat with a young rat by sharing the same circulation system, the old animal benefits from the blood of the young animal. But even more so, the young animal suffers from the blood of the old animal."
The implication is that aging blood contains factors that actively drive aging — not just an absence of youth factors but the presence of age factors. Identifying and neutralizing those factors is a legitimate and active field of research.
What followed the publication of these results, however, was immediate exploitation. "Right from the time when these scientists at Stanford published their first finding, they started getting creepy phone calls from rich people about where could they get blood. And companies started sprouting up getting blood from young donors and selling them at a huge markup to rich old people." The transfusions, Ramakrishnan notes, had almost no effect — because the mechanism requires an actual shared circulation over time, not a single infusion.
IGF-1, Insulin Signaling, and the AMPK Axis
Caloric restriction and mTOR are part of a broader nutrient-sensing network. Ramakrishnan points to IGF-1 (insulin-like growth factor 1) as another major arm of the same system: "There are many important biochemical pathways affected by caloric restriction. One of them is the IGF-1, insulin growth hormone factor."
IGF-1 and insulin signal abundance — they push cells toward growth, division, and protein synthesis. When they are chronically elevated (as in the modern high-calorie diet), they keep the cellular machinery in a growth-forward state that accelerates aging. In worms and mice, mutations that reduce IGF-1 signaling reliably extend lifespan — among the most reproducible findings in all of aging biology.
On the opposite side of this axis sits AMPK (AMP-activated protein kinase) — the cell's low-energy sensor. When glucose is scarce (during fasting, caloric restriction, or exercise), AMP levels rise and AMPK activates. It directly inhibits mTOR and switches the cell into a conserving, repair-oriented mode: turning on autophagy, improving mitochondrial biogenesis, reducing inflammation. AMPK is also one of the main targets of metformin, which partly explains why the diabetes drug keeps appearing in longevity discussions. While Ramakrishnan does not mention AMPK by name in his lectures, it is the molecular engine underneath several interventions he does discuss — fasting, exercise, and metformin — all of which converge on this same energy-sensing switch.
"The whole point is not to fast just to restrict calories. Intermittent fasting — that's the starvation mechanism biochemically. You hijack what would be the starvation mechanism... so you can have your cake and eat it."
Proteostasis: When the Protein Factory Fails
This is the pathway most directly connected to Ramakrishnan's own Nobel Prize work. The ribosome is the machine that builds every protein in every living cell — and ribosome function is central to what he calls proteostasis: the cell's ability to maintain a clean, correctly-folded inventory of proteins.
"This phenomenon of proteostasis — preserving the correct expression of proteins in the cell and maintenance of the orchestra of proteins in the cell — being connected with aging... the evidence built up over years, and you would just sort of watch it happen."
As cells age, several things go wrong with protein quality control simultaneously. Ribosomes become less accurate — they make more errors during translation, producing malformed proteins. The proteasome (the cell's protein-recycling machinery) becomes less efficient at tagging and destroying defective proteins. The unfolded protein response — a stress system that senses misfolded proteins and triggers repair — becomes chronically activated rather than episodically activated.
The result is a slow accumulation of damaged, misfolded proteins that clog cellular machinery and generate toxic aggregates. This is not just abstract biology: the amyloid plaques of Alzheimer's disease and the protein aggregates of Parkinson's are the clinical endpoint of proteostasis failure.
The proteasome
Senses defective proteins via ubiquitin tags and destroys them. Slows with age. Caloric restriction and exercise both help maintain proteasome activity — another reason those interventions keep showing up in longevity data.
The ribosome connection
Ramakrishnan's career was built on ribosome structure. His pivot to longevity was not a departure — it was a continuation. Ribosome error rates rise with age, and accurate translation is foundational to proteostasis.
Reactive Oxygen Species: The Mitochondrial Exhaust
Mitochondria are the source of most cellular energy — and most cellular damage. Because they handle oxygen chemistry at scale, they inevitably produce reactive oxygen species (ROS): partially reduced oxygen molecules that are chemically aggressive and will damage whatever they encounter — proteins, lipids, DNA.
"Mitochondria are a center for oxygen usage, and they can create what are called free radicals or reactive oxygen species — these are partially reduced oxygen atoms which are chemically very reactive. They can cause a lot of damage."
For decades, the prevailing theory was that ROS accumulation was the primary driver of aging (the "free radical theory of aging"), and that antioxidants were the answer. That theory has largely not held up. Supplementing with antioxidants does not reliably extend lifespan in humans, and in some trials it has made things worse — possibly because low-level ROS also serve as signaling molecules that stimulate the cell's own repair systems. The relationship between oxidative stress and aging is real but more complex than simple accumulation.
What does work is maintaining mitochondrial quality — through mitophagy (the recycling of damaged mitochondria), regular exercise (which stimulates new mitochondrial biogenesis), and caloric restriction (which reduces the metabolic load and ROS production rate).
Cellular Reprogramming: The Yamanaka Frontier
The most speculative but genuinely exciting pathway Ramakrishnan discusses is cellular reprogramming — the ability to reset a cell's epigenetic age without changing its identity.
"Yamanaka showed that if you were to introduce just four factors — these are genes for proteins that regulate other genes — you could take a fully differentiated cell like a skin cell or liver cell or heart cell and make it go backwards in development all the way back to what's called a pluripotent stem cell. Pluripotent means it can make any tissue."
The four Yamanaka factors (Oct4, Sox2, Klf4, c-Myc) don't change the DNA sequence — they reset the epigenetic modifications layered on top of it. A fully differentiated adult cell, when exposed to these factors, loses its identity and returns to an embryonic-like state. The epigenetic clock, which normally only moves forward, gets wound back.
The therapeutic insight is that you might not need full reprogramming — just partial, transient reprogramming that resets the epigenetic age of cells without erasing their identity. This is called partial reprogramming, and several biotech companies (including one backed by Jeff Bezos) are now pursuing it as a therapeutic strategy. The risks are significant — full reprogramming can trigger cancer — but the target is clear: reset the clock without losing the function.
Yamanaka factor reprogramming in living organisms can cause teratomas — tumors that grow hair, teeth, and other tissues. The challenge of partial reprogramming is applying enough of a reset to reverse aging without triggering uncontrolled cell proliferation. This is the central engineering problem the field is working on.
Exercise: The One Thing That Actually Works Right Now
After walking through every cutting-edge pharmaceutical pathway, Ramakrishnan keeps returning to something that requires no clinical trial and costs nothing:
"The sort of Trinity for health — which is something possibly our grandparents told us — which is to eat more moderately and eat healthy diets, to get enough sleep, and to get exercise. It turns out that each of those affects the other two. This Trinity, if you do all of them at once, works better than any medicine on the market and has no side effects and it's free."
He is not being folksy. He is making a precise scientific claim: the combination of moderate diet, sleep, and exercise activates more longevity pathways simultaneously than any single drug being studied. Exercise alone activates AMPK, stimulates mitophagy, promotes new mitochondrial biogenesis, reduces chronic inflammation, clears senescent cells, improves proteostasis, and maintains stem cell pools. No pharmaceutical can currently match that profile.
The irony he notes: he does all three himself, and he still developed high blood pressure and high cholesterol requiring medication. The Trinity reduces risk; it does not eliminate it. But as a baseline before pharmaceutical interventions, nothing else comes close.
What Ramakrishnan Actually Believes
After surveying all of this, the question is: what does a careful scientist make of it?
His position is neither dismissive nor credulous. He believes the underlying biology is real — these pathways genuinely matter for aging, and understanding them is already producing insights that will eventually translate into medicine. He is genuinely excited about senolytics, cautiously interested in rapamycin dose optimization, and honest about the still-open questions around NAD precursors.
But he keeps coming back to one theme: the field needs large-scale clinical trials on healthy people, and those trials take time. "You need large-scale trials on healthy people, not on diabetics who have to take metformin." Most of what we know comes from model organisms. The leap from a worm or a mouse to a human is not a given.
And his most important distinction — the one that gets lost in the longevity marketing — is between healthspan and lifespan:
"If scientists find out ways to extend lifespan in a healthy way — nobody wants to extend lifespan and just be miserable for 30 years — but if they can extend the number of healthy years that we live, it'll be impossible to stop." The goal is not simply more years. It's more good years. That framing changes which interventions you prioritize and how you measure success.
The pathways are real. The interventions are early. The hype is loud. Ramakrishnan's value is that he can read the biology clearly enough to tell the difference — and he's honest about how much remains unknown even to the experts who are building this field in real time.
